I ask questions, follow inconsistencies, look across departments and compare what the data says with what is actually happening. The goal isn't to confirm the existing theory. It's to find the root of the problem.

Because a process can look fine on paper and fail in practice. A KPI can improve while the underlying issue gets worse. And the department being blamed may simply be where someone finally notices the problem.
